The Office of Student Accountability and Conflict Transformation receives and investigates referrals for violations of the Student and Organizational Codes of Conduct. 

Report an Incident

All other reporting options listed are directed to the appropriate Moravian University departments, offices, or committees and may not operate under the same processes and policies as the Office of Student Accountability and Conflict Transformation. 

Please remember not to use these forms if there is an immediate threat to yourself or others in the campus community.

General Concern Form

Use this form to report a complaint or concern you would like the University to resolve. Including but not limited to violations of the Student Code of Conduct and the Organizational Code of Conduct.

Title IX Form

Use this form to make a report to the Equal Opportunity & Title IX Coordinator if you experience or witness an incident of protected class harassment/discrimination or an incident of sexual violence.

Bias Incident Reporting Form

Use this form to report incidents of bias that you witness or experience.

SHARE Reporting Form

Use this form to report concerning behaviors or incidents in order to make the University aware of students who may need additional support or intervention.

Campus Police

If you need to report a concern to Moravian University Police, please use the form on their website.
